Serious Medical Condition Program
If you or someone in your household relies on electrically powered medical equipment such as an oxygen machine, dialysis machine or heart monitor, A&N Electric Cooperative should have a current Serious Medical Condition Certification Form on file. This form, certified by a licensed physician, allows us to identify your account and document your medical needs.
To enroll in the program, please complete the Serious Medical Condition Form (English) or Serious Medical Condition Form (Spanish). If applicable, a legal guardian or power of attorney may complete the form on your behalf. Be sure the physician certification section is completed before submitting the form.

Important Information
While A&N Electric Cooperative makes every effort to restore power as quickly and safely as possible, we cannot guarantee uninterrupted electric service or predict how long an outage may last, particularly during major storms or widespread damage.
If you depend on electrically powered medical equipment, we strongly encourage you to have an emergency preparedness plan. This may include:
- Having a backup power source, such as a generator.
- Understanding the battery backup or emergency features of your medical equipment.
- Speaking with your healthcare provider about additional resources or assistance available during extended outages.
If we anticipate a prolonged outage, we will make every reasonable effort to contact enrolled members by telephone. We will also attempt to notify participants in advance of planned outages whenever possible.
